When we receive your completed form, you will receive a welcome pack detailing the services available to you. If you have requested contact by phone, one of our Carer Wellbeing Team will contact you to talk to you about your caring situation and tell you about the services available to you.

Any personal or sensitive information, such as health and ethnicity, that is provided to Carers Support West Sussex, or shared by you may be recorded and is used for the purposes of providing advice, information and support to you in your caring role.
