Hospital Team Support Officer (18 Month FTC)
Work Location: Horsham - Home Based
Salary: £15,647.00 (FTE £26,316.00)
Work Pattern: 22 hours worked across 4 or 5 days. To be discussed at interview

Are you passionate about teamwork and the supporting the wellbeing of others?
We are seeking a dedicated and organised Hospital Team Support Officer to become a vital part of our Hospital Team who provide a highly valued service to unpaid family and friend carers where they or the person they care for are a patient in hospital. The team work across 5 Acute and 8 community hospitals serving the residents of West Sussex.
The successful candidate will be responsible for providing administrative support to the team and liaising directly with carers to ensure they are able to access the support they need swiftly and without any unnecessary barriers.
The role is mainly remote / homeworking with the expectation that you may need to travel on occasion to in-person meetings and training across the county. This role would therefore suit those located in, or close to, West Sussex.

Job Role
- Triage: Oversee and organise the Hospital Team action list and manage referrals to the Hospital Service, liaising with the referrer if required.
- Co-ordinate Appointments: Where required contact carers to arrange appointments for one-to-one support on the phone or on-site in one of the hospitals served, ensuring carers can access the service easily and in a timely manner.
- Referrals: Manage new referrals into our Support at Home after Hospital service, liaising with the referrer if required. Add new carers to the database and call carers to arrange an appointment time for assessment.
- Demonstrate Impact: Support the collection and maintenance of data for reporting. Showcase the difference the service makes in carers’ lives through direct conversation to gather carer stories, evaluations and feedback.
