Are you passionate about making a difference and the supporting the wellbeing of others?

We are seeking a dedicated and organised Wellbeing Worker to become a vital part of our Hospital Team. We provide a highly valued service to unpaid family and friend carers where they or the person they care for are a patient in hospital. The team work across 5 Acute and 8 community hospitals serving the residents of West Sussex.

The successful candidate will be responsible for providing support and guidance to carers helping them navigate their hospital journey, working closely with medical staff and other professionals.  

As part of the team this role provides a time-bound Support at Home after Hospital service to eligible carers to help with the transition home.

You will have excellent problem-solving skills, able to take a proactive approach to overcoming obstacles and ensuring support for individuals navigating complex systems. You will be a natural communicator with excellent administrative skills and a strong attention to detail.

This hybrid role combines remote work with 2 days per week working from Royal Sussex, Princess Royal or local Community Hospitals plus occasional travel to in-person meetings in West Sussex.

Job Role

Carer Support: Provide a blend of in-person, on-line and telephone support to carers where they, or the person they care for is a patient in hospital. Proactively reach carers from under-represented and diverse communities ensuring our service is accessible.

Information & Guidance: Help carers to navigate the hospital systems and connect with teams and other services to access appropriate support ensuring carers feel included in their hospital journey.

Partnerships: Work collaboratively with health, social care and voluntary sector partners, seeking out opportunities to work together to enhance support available to carer during and after hospital discharge.

Support at Home after Hospital: Following hospital discharge, provide support to carers for up to six weeks. Use the Carers StarTM to identify priorities and help to put in place services and support to support the transition home.
